Job Summary In this position, the Social Worker will provide comprehensive psycho‑social assessment, management, and support to at‑risk patients and families at Upstate Medical University Hospital. The role covers the entire downtown campus during evening hours and includes weekend rotation coverage to support throughput in the Emergency Department and inpatient units during high volume times. The Social Worker will actively manage patients through collaboration, coordination, monitoring, evaluating options, and identifying required services to meet individual psychosocial needs. Responsibilities include consultation, assessment, documentation of brief counseling, interventions, discharge coordination, and referrals to meet the psycho‑social needs of patients and families. The Social Worker will participate in departmental meetings, in‑services, education, and quality improvement activities, and will perform other relevant duties, including coverage for other social workers and backup responsibilities. Minimum Qualifications M.S.W., L.M.S.W., L.C.S.W. or eligible 3 years relevant post‑master's experience (preferably in a hospital and/or healthcare environment) New M.S.W. graduates who are L.M.S.W. eligible will be considered for an entry‑level (SL2) position at a commensurate salary after obtaining a Limited Permit within 12 months of receipt of the limited permit. Preferred Qualifications Post‑MSW relevant experience Work Days Days and evenings with every other weekend rotation.
